I wasn't disappointed with the ending; a lover's worst fears are played on and it will leave you feeling uneasy. Any movie that can do that deserves some respect.

not the film you think it is . . .
I wasn't sure about this film initially, but by the end I was caught. The layers of reality and distortion, which may seem heavy handed at points, are, by the end, demonstrated to be subtle, and powerfully executed. Not the film you think it is.

Back to back, I watched this along with the French thriller "Haute Tension". I didn't think the two movies had very much in common so imagine my surprise when they both had essentially the same ending. Come to think of it, I believe I've seen at least a dozen movies in the last decade with this ending. At least a dozen. I should be more surprised when movies don't end this way. "Shutter Island" is okay, it LOOKS great, but Scorcese is slumming and for heaven's sake, could Hollywood scribes PLEASE think of a new twist ending. 3 stars.

How could you say "Hitchcock never would have shot so much film that didn't move the plot forward"? Only if your not keen enough to clue into the fact the entire film was foreshadowing in of it's self. This type of film you intentionally can't "move the plot forward" as "jcphil" would like because there would be no movie than. You could "move the plot forward but than it would be a 5min. story without the entertainment of the surprise.
If you liked "The Game" than you must see this. Wasn't as much of a surprise as that one (I called it early in the film, the real plot that is.) But still fun to have that ahha of what everything meant prior to the end.
Cinematography was a little off, but I'm sure it was intend rather than cheap effects. Like the old school people ridding in a car and the background is obviously rolling separate of the in focus actors.

In the end, there are some satisfying things about it. But the mental hospital setting has been done to death and it doesn't get revived here. I kept looking for genuinely tense moments and not finding them. The noir elements also seem overdone. DiCaprio seems stalled here and relying on method to carry him through. He was so much sharper in Inception.

Dont bother
I agree with the review "Scorsese: Stick to gangster movies". The look of the movie was fine. The story and dream sequences...I don't think so. I couldn't wait for it to end!

8/10Great
Most people, including those I saw this film with, would rate "SHUTTER ISLAND" three stars, but I think it's better than that -- it creates a dark, ominous atmosphere that I found unique and suspenseful. The setting contributes a lot to that atmosphere -- all the action takes place during an horrific storm, mostly at night, on an isolated island, at a facility for the criminally insane, where awful medical experiments may be taking place. Our hero, a US marshall, and his new partner are steaming toward the island to investigate the escape of a patient. When they arrive, they get everything but straight answers from the doctors and staff. Our hero begins to have flashbacks both to his days as an infantryman liberating Dachau, where he saw first-hand what people are capable of doing to others, and to his dead wife, who keeps reappearing to offer clues about the investigation. He seems to have an agenda beyond his job as a marshal. We get pulled into his psychosis and have to puzzle that through to solve the present mystery of the island. The director gives us clues but also mis-directions, and the ending comes as quite a surprise. The acting is superb all around, and the film is beautifully shot and quite well done. It's excellent at creating and maintaining this spooky, dread atmosphere, which was enough for me to enjoy it. It may not be the best psychological thriller or suspenser you'll ever see, but it rises far above the pack.
